Download regulations to establish the procedure of the material aid program

1) Program Application duly completed.
2) 2×2 photo of the applicant and spouse.
3) Photocopy of the deeds or evidence that indicates you are the owner of the property.
4) Evidence of income for anyone over 18 years of age who resides in the household.
5) Nutrition Assistance Certification (PAN)
6) Unemployment Certification
7) ASSUME Certification
8) Social Security Certification
9) Copy the Social Security card of everyone who lives in the house.
10) Copy of photo identification of anyone over 18 years of age.
11) Water, Electricity or Telephone Receipt in the name of the applicant.
12) Estimated Materials of the affected area.
13) Provide written evidence of the agreement with the contractor who will perform the labor.
14) Photo of the house
15) Three (3) quotes certified by the hardware stores on the ASG list.
